Abstract

Spinal bio-mechanical radicular pain has become a common chief complaint in our health care system.

Keeping in mind that it can represent a wide category of etiologies ranging from a self-limited / uncomplicated sciatica, to a progressive radiculopathy leading to motor deficit. As such, it demands a comprehensive diagnostic evaluation, and a treatment plan, accordingly.

It is therefore empirical to have a clear understanding of the specific mechanism, neurological manifestation, and the prognosis of the contemplated treatment procedure.

In this presentation, specific diagnostic criteria, including the neurological presentation and its correlation with advanced imaging studies have been elaborated.

More specifically, variations of neurological presentation at different stages of the closed reduction protocol have been emphasized.

Unique to this presentation, the contribution of the CNS to the ridiculer complaint, such as centralization / peripheralization, have been discussed in detail, with emphasize on prognosis correlation.

The phenomenon of silent – progressive neurological deficit which can significantly alter the prognosis has also been pointed out.

Systematic re-evaluation procedures during treatment protocol have been discussed in detail, including specific red flag indications.

Inter-segmental stability as the main criteria for selecting the appropriate treatment protocol

has been reiterated.
